Student Withdrawal (Exit Form) — Royal Oak Schools
Students who are leaving Royal Oak Schools (except current-year seniors still active in June)
are required to have a Student Withdrawal/Exit Form completed. Counselors submit the
appropriate end-of-year exit codes based on information and documentation provided.
What families should provide
- Building designee/counselor with the student’s last day of attendance
- Reason for leaving and notification method
(for example: records request, documented parent call, written notice, etc.)
- New school/district information (if applicable)
- Parent signature (if available)
For school use only
The form includes exit codes that identify the reason for exiting (such as graduation,
transfer out, moved out of state, dropout, home school, non-public school, etc.),
along with MIStAR processing fields completed by staff.
